Registered Nurse-Diabetic Educator

The Diabetes RN Care Manager is responsible for the administration and development of core diabetes, nutrition, and isometrics related to education and training programs for staff, veterans, and families. The employment of a variety of methodologies to assess, diagnoses, plan, implement, and evaluate needs that addresses system wide goals. This individual will use the group process to analyze, resolve problems, and improve outcomes in diabetic and pre-diabetic populations. Serves as a consultant/resource to others regarding patient care and outcomes and collaborates with other members of the care team and the community as a content expert. Educates clinical staff, peers, trainees and residents in regards to diabetic care/management, nutrition, and physical science principles. Education may include protocol initiation/modification, diabetes outcome awareness/evaluation and program outcome awareness/evaluation. Possesses the knowledge and skills to effectively apply all aspects of the nursing process with a collaborative, interdisciplinary practice setting to facilitate the effective care of patients with diabetes and pre- diabetic populations. Works with all disciplines; Nutritionist, Social Workers, Nurse Practitioner, Endocrinologist, Clinic Nurses and Clerks, and to facilitate accreditation and regulatory standards related to diabetic care and veteran wellness. Responsible for the development of programmatic strategies, interventions and evaluating procedures and processes to induce effectiveness, innovation and efficiency of their program. a. Describing the diabetes disease process and treatment options b. Incorporating nutritional management into lifestyle c. Incorporating physical activity into lifestyle d. Using medication(s) safely and for maximum therapeutic effectiveness. e. Monitoring blood glucose and other parameters and interpreting and using the results for self management decision making. f. Continuous Glucose Monitoring and Insulin Pump overview and data collecting. Preventing, detecting, and treating acute complications h. Preventing, detecting, and treating chronic complications. Developing personal strategies to address psychosocial issues and concerns j. Developing personal strategies to promote health and behavior change. The RN will collaborate and communicate program needs, action plans, and performance measures throughout the organization and with diverse teams. Assist with collecting and analyzing data related to this patient population, report findings and make recommendations for improvement. The RN is a self-directed professional who assesses plans, implements, coordinates, monitors and evaluates client (i.e., patient,family and other disciplines) care activities, while ensuring and facilitating the achievement of quality in a cost effective manner. Identifies the Veterans with diabetes who do not meet diabetes A1C guidelines (>8.0% or have not had an A1C in 365 days or greater). The American Diabetes Association recommends that people with diabetes that are meeting treatment goals and have stable blood glucose levels have the A1C test twice a year. Follow up and close gaps in care, using the telephone to call patients with A1C's 8.0% and intervene by telephone to engage in care. Evaluate the need for Home Telehealth (HT) monitoring, refer to HT, involve HT nurse in home monitoring and communication with the PACT team. Evaluate the need for medication adjustment and refer to the PCP, Endocrinologist, or Diabetes Nurse Practitioner. Works with the Endocrinologist's concerning diabetes education, patient groups, etc. Assumes the leadership role in facilitating the care of assigned High risk Diabetic patient caseload. Participates as a member of the interdisciplinary healthcare team to facilitate the continuum of care and education of patients with diabetes. Assesses the diabetic patient and pertinent family members' knowledge of diabetes and develops and implements a patient-centered care plan. Monitors and evaluates resource utilization, patient outcomes, patients' skills in self-care, and ability/readiness to learn. Communicates with providers regarding patient needs and ongoing status of the plan of care. Provides clinical resource and consultation to staff and demonstrates advanced clinical competence in diabetic care. Maintain professional knowledge and skills based on best practice and evidence-based information, such as professional journals, workshops and seminars. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Experience:
Bachelors of Science in Nursing; Certification in Diabetes Education; Knowledge of Insulin Pumps. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
